JOB TITLE: ASC RN Scrub/Circulating

SUPERVISOR: Director of Surgery Centers

HOURS PER WEEK: 40hrs/Non-Exempt (4 x 10hr shifts - This position is estimated to Scrub 20hrs/wk and to Circulate 20hrs/wk)

DEPARTMENT: Day Surgery

The following information is designed to outline the essential functions and position requirements of this job. It does not identify all tasks that may be expected, nor address the performance standards that must be maintained. 

HIRING BONUS: $1,000 after 90 days 

PRIMARY FUNCTION:  To provide quality care and adequate evaluation and monitoring of legal and physical needs of patient undergoing surgical intervention. Assists the surgeon by providing and controlling the sterile instruments, supplies and equipment required during surgical intervention.  Assure instruments and supplies are cleaned, sterilized and available when needed.

DUTIES AND RESPONSIBILITIES:  (*ESSENTIAL FUNCTIONS) TPC123
1.    Check the wristband of patient, confirm patient identity. *
2.    Check the chart for completed procedural consent and verify with patient. *
3.    Remain available for questions throughout process. *
4.    Complete short-term patient teaching, preparing patient for procedure and post-operative care. *
5.    Be vigilant as to the change in condition, with immediate assessment, interpretation and documentation of signs and symptoms, notifying surgeon of any changes. *
6.    Receive oral post-operative report from anesthesia personnel.  This will include procedure, type of anesthesia, medications used, general condition of patient and any other pertinent O.R. information. *
7.    Note level of consciousness, complete thorough assessment and document in EMR as required by policy. *
8.    Maintenance of safety*
     a.    Participate in team time outs to ensure all elements complete including instrument sterility double check.  
     b.    Remove all outer instrument containers before patient enters if possible.
     c.    Assure that sponge, needle, and instrument counts are correct.
     d.    Position the patient keeping in consideration:
          i.    Functional alignment.
          ii.    Exposure of surgical site.
          iii.    Maintenance of position throughout procedure.
     e.    Verify safe zone with tech during time outs.
     f.    Apply grounding device to patient, if necessary.
     g.    Provide physical support.
9.    Physiological Monitoring*
     a.    Calculate effects of patient of excessive fluid loss.
     b.    Distinguish normal from abnormal cardiopulmonary data.
     c.    Report changes in patient’s pulse, respirations, temperature, and blood pressure.
10.    Psychological Monitoring*
     a.    Provide emotional support to patient.
     b.    Stand near/touch patient during procedures/induction.
     c.    Continue to assess patient’s emotional status.
     d.    Communicate patient’s emotional status to other appropriate members of the health care team.
11.    Nursing Management*
     a.    Provide physical safety for the patient.
     b.    Maintain aseptic, controlled environment.
     c.    Effectively manage human resources.
     d.    Complete documentation in a timely and accurate manner.
12.    Follows rights of medication administration, appropriately signs out medications.
13.    Open sterile supplies and prepare for each operative procedure. Whenever possible, remove instrument containers before the Patient enters the room. *
14.    Compile and make available all necessary supplies needed for procedures.  *
15.    Prepare surgical trays, instruments, equipment, and supplies before and during procedures.*
16.    Assist and anticipate the needs of surgeons during procedure according to prescribed standards. *
17.    Set up table, instruments and supplies according to the type of procedure. Double-check instrument sterility and verify with circulator. *
18.    Establish “safe zone” to avoid sharps injuries and communicate to team.  
19.    Actively participate in timeouts. *
20.    Maintain correct count of sponges and needles as directed. *
21.    Maintain aseptic technique at all times as dictated by accepted standards. *
22.    Assist in maintenance and care of specimens as directed. *
23.    Assist in application of dressings, casts, and in moving patients as directed. *
24.    Help to clean and maintain rooms, equipment, and furniture according to prescribed procedures. *
25.    Assist with emergency procedures as directed. *
26.    Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.  Demonstrate knowledge of the principles of growth and development over the life span, and is able to identify and categorize each patient’s age-specific grouping of needs. *
27.    Work in or assist with Sterile Processing department as needed.
28.    Adherent to current standards of practice/infection control as developed by agency standards. *
29.    Responsible for the proper cleaning and sterilization of individual instruments/sets per manufacturer recommendations. *
30.    Maintain and update instrument sets and reusable sterile supply lists. *
31.    Ensure all loaner instruments/sets are properly identified and cared for and are returned to the proper institution/vendor in clean and/or sterile condition. *
32.    Immediately remove any damaged instrumentation from service, and communicates such with appropriate individuals. *
33.    Maintain all autoclaves, washer-decontaminators, load documentation. *
34.    Participates in cleaning and upkeep of washers and autoclaves. *
35.    Monitor and document all biological testing for autoclaves and Sterrad machine. *
36.    Immediately report any discrepancies or positive results in required testing to the appropriate individual. *
37.    Assist in maintaining prescribed levels of sterile and non-sterile supplies for the operating room, as assigned. *
38.    Participate in preparation of the next day case cart supplies in a timely manner. *
39.    Participate in timely delivery of instruments/supplies for in-progress cases as needed. *
40.    May assist in the transport of instruments from the operating room to dirty utility room in a timely manner to assure all instruments are cleaned and sterilized before the day they are needed. *
41.    Assist in the turnover of operating rooms, as needed.
42.    Maintain current knowledge of all policies and procedures. *
43.    Uses appropriate PPE at all times and other settings where required.. *
44.    Attend staff meetings. *
45.    Uses appropriate use of PPE for patient care and other settings where required. *
46.    Be an active participant in committees (i.e. Safety Committee, Infection Control). *
47.    Participate in orienting new staff. *
48.    Maintain regular work attendance and punctuality.*
49.    Understand hazard risk and demonstrate complete understating of established emergency procedures for the Surgery Center. *
50.    Reports incidences or issues through chain of command and maintains confidentiality.  Complete all employee/patient incident documentation in a thorough and timely manner.*
51.    Assist/floats to other departments as needed.
52.    Employees will not be discourteous or disrespectful to a customer or any member of the public while in the course and scope of company business.*
53.    Work in a cooperative manner with management/supervision, coworkers, patients and vendors.*
54.    Abide by company policies.*
55.    Demonstrate competent usage of proper body mechanics in all activities. *
56.    Other duties as assigned.

REQUIREMENTS:
⦁    Graduate of a Professional School of Nursing
⦁    Valid Oregon Nursing license
⦁    Current CPR/BLS & ACLS certification
⦁    Previous surgical experience
⦁    Previous Surgical Scrub experience
