JOB TITLE: Director of Population Health

SUPERVISOR: Chief Medical Officer

HOURS PER WEEK: 40/Exempt (Position may become eligible for remote work)

DEPARTMENT: Administration

HIRING BONUS: $1,000 after 90 days 

PRIMARY FUNCTION: To provide leadership to The Portland Clinic’s organizational commitment to practice transformation, continuous quality improvement and value-based, patient-centered care

DUTIES AND RESPONSIBILITIES: (*ESSENTIAL FUNCTIONS) TPC123
1.    Responsible for value-based care initiatives (quality metrics, care management, utilization, cost, patient experience, reporting, etc.) and related payer strategy 
2.    Provide regular performance reporting to key stakeholders (staff, providers, leadership) 
3.    Oversees the population health team and all associated programs and workflows. 
4.    Lead the Population Health team, including care navigators, Medicare services representative, RN and SW care managers and TOC (transition of care) RNs. 
5.    Responsible for all strategy and operations related to value-based care, and accountable for all related metrics.
6.    Provide leadership for the development, implementation, and evaluation of initiatives to improve patient outcomes through preventive care and management of chronic conditions.
7.    Continuously monitor and evaluate care management program objectives, processes, and workflows to ensure the program is meeting payer and patient centered medical home requirements, as well as the needs of The Portland Clinic’s providers, staff and patients.
8.    Serve as liaison to payer partners with whom The Portland Clinic is involved in value-based contracts.
9.    In collaboration with the finance team, manage accountability for and report on value-based operational performance metrics, payments, and areas of opportunity.
10.    Direct development of actionable value-based care performance goals and related improvement projects.
11.    Collaborate with internal and external stakeholders to create effective information and analyses to optimize population health, value-based care performance and strategic objectives.
12.    Develop and implement processes for data collection and exchange with payer partners.
13.    Be the defined point of contact for providers, staff and external partners related to communication for quality metrics, population health, and value-based care.
14.    Contribute to The Portland Clinic’s ongoing contracting strategy to further the clinic’s involvement and success in value-based contracting and actively participate in payer contract negotiations.
15.    Prepare and deliver presentations related to quality, value-based care, risk adjustment, etc. for provider, staff, and leadership teams.
16.    Lead councils and committees related to quality and value-based care, including but not limited to Quality Management Committee (QMC),  PCCA Quality and Utilization Subcommittee.
17.    Facilitate identification, adoption, implementation, and utilization of “best practice” clinical practice guidelines and engagement protocols for preventive services and management of specific diseases/conditions.
18.    Oversee The Portland Clinic’s patient centered medical home (PCMH) designation and all associated requirements.
19.    Assist the Chief Medical Officer and/or Medical Directors with provider education and engagement, as needed.
20.    Handle all appropriate communications, whether written or verbal in a courteous, professional manner.*
21.    Attend and participate in professional organizations and meetings to enrich the value-based care improvement efforts and performance at The Portland Clinic.
22.    Maintain ability to prioritize work and meet deadlines.*
23.    Coordinate, facilitate and participate in clinic wide quality and population health education and activities.*
24.    Participate as an active team member in a patient-centered medical home.
25.    Employees will not be discourteous or disrespectful to a customer or any member of the public while in the course and scope of company business.*
26.    Work in a cooperative manner with management/supervision, coworkers, customers and vendors.*

REQUIREMENTS:
⦁    Graduate of a Professional School of Nursing
⦁    Valid Oregon Nursing license
⦁    Knowledge of QI/QA processes and concepts
⦁    Strong background working with and improving clinical quality metrics, as well as other health plan quality metrics.
⦁    Demonstrates a deep understanding of care delivery and quality improvement and has a successful track record of improving quality metric performance and delivering quadruple aim goals.
⦁    Demonstrates understanding of provider reimbursement and alternative payment models.
⦁    Presents a positive and professional image internally, with providers, and more broadly in the communities we serve.

PREFERENCES:
⦁    Two years of previous experience in a Medical Clinic
⦁    Excellent written and oral communication skills
⦁    Computer experience
⦁    Strong problem-solving skills
⦁    Strong organizational skills
⦁    Knowledge of, and previous experience with, QI/QA processes and concepts
⦁    Possess the temperament to confront peers and foster conflict resolution
⦁    Good listening skills
