Responsibilities:

  • Work with individuals with a mental health diagnosis to enhance their recovery
  • Work with patients to address barriers to care, increase motivation, and establish professional, dynamic relationships with patients, providers, referral sources, and other individuals
  • Coordinate with interdisciplinary team to ensure a warm hand-off for next level of care
  • Document services in an electronic medical record system
  • Requires rotating on-call shift coverage that includes evening, weekends, and holidays
  • Work with other members of the Mobile Crisis Response Team to respond in our catchment area (Pilsen, Little Village, Lawndale)

The CRSS will use their experiences in mental health recovery to support individuals in various stages of crisis, with support from MCR team members; as well as link individuals to appropriate levels of aftercare. CRSS are “on call” on a rotating schedule 24 hours/7 days a week and receive additional compensation for their time on call. This job requires rotating on-call shift coverage that includes evenings, weekends and holidays. This position is part of the mobile crisis response team in the community during assigned shifts which will cover 24/7/365 days of the year.

Must have at least one (1) year of experience in the substance use field;

Medication Assisted Treatment knowledge such as Methadone and Suboxone (preferred)
