Summary of Job

Under the direction of the CHW supervisor, the CHW is responsible for providing and coordinating health education and disease management education, assisting individuals with navigating the health and social care systems, and providing case management services (under supervision). The CHW will work within an interdisciplinary team and will serve as a bridge between the patient and the medical system by building trusting relationships with community members served by the health center or program(s). Home visits may be required for some interventions. The position is grant funded. SUHI is seeking an enthusiastic, passionate CHW candidate who meets job requirements outlined below.

 

Position Requirements:

  • Must have high school diploma or GED
  • Two years of health education experience preferred
  • Candidates residing in a South Side community are preferred
  • Passion for and dedication to working with community members and providers
  • Punctual, reliable and willing to learn
  • Demonstrated commitment to racial and health equity
  • Proficiency in Microsoft Word, Excel, and PowerPoint preferred

 

The pay range for this role is $18.85- $32.80.
