It is reasonably expected that employees may have occasion, during the performance of their job, to drive Company or personal vehicles.  For this reason, the answers to the following questions are required by all applicants. The lack of a valid driver’s license will not necessarily preclude employment in those positions where driving is not an essential function.

Have you had motor vehicle violations including license being suspended or revoked, DUI or other moving violations within the past seven (7) years?

Matters of public record are defined as records documenting an arrest, indictment, conviction, civil judicial action, tax lien, or outstanding judgment.

I understand that such public record information generally must be disclosed to me within seven days of the date the information is received, regardless of whether it is received orally or in writing.  I understand that I may waive my right to receive such information.
