Within our Community Connections Division, and in partnership with the Florida Department of Health, Human Services provides the Pinellas County Health Program, offering prevention focused health care, mental health and substance use services, specialty care referrals, and case management services to qualified low-income residents.  We are seeking a dynamic team player with a passion for helping others to fill our Case Manager 1 position. 

The Case Manager 1 is a professional office/field case management role whose primary responsibilities include processing applications to determine program eligibility and facilitate enrollment into the County Health Program; providing short term case management services to applicants, including making referrals and helping connect applicants with community resources; and responding to in person and phone inquiries from the general public seeking information and resources. 

Our Case Managers work in various sites throughout the county to increase accessibility to our services.  Employees in this class exercise considerable independent judgement throughout the eligibility process, in providing guidance and resources for clients, and in establishing and maintaining working relationships with our community partners.  A high degree of public contact is exercised, requiring professionalism, tact and poise.  This position requires an ability to relate professionally to a wide range of diverse residents applying for public assistance. 

This Human Services Case Manager 1 (Eligibility Case Manager 1) position is located at:  647 1st Ave N, St. Petersburg, FL 33701.  Work Schedule:  Monday – Friday; 8:00 a.m. – 5:00 p.m.   Hybrid schedule including office, outposts, and some remote work.

What Will You Do?

  • Screen and interview applicants in person and over the phone, and responds to questions regarding availability of, and eligibility for benefits.
  • Process applications to determine eligibility for medical assistance, advises applicants of needed documentation, and refers ineligible applicants to alternate community resources for assistance.
  • Provide information and referral services to clients.
  • Work with community partners and other agencies to assist clients in accessing services and programs.
  • Answer phone calls from the general public to provide information and resources.
  • Maintain an active caseload for eligibility and short-term case management.
  • Enter accurate and timely notes, and scans documentation and forms in appropriate databases.
  • Apply policies and procedures of the department's eligibility determination process.
  • Perform data entry and record keeping.
  • Prepare and prints medical ID cards and issues to clients.
  • Attend meetings and trainings.
  • Perform other related job duties as assigned.

What Do You Need?

Experience:  Technical and professional experience in social/human services case management or related field.

Degree: Social Work, Psychology, Sociology, or a related field.

  • 5 years of experience as described above; or
  • Associate's degree and three (3) years of experience as described above; or
  • Bachelor’s degree and one (1) year of experience as described above; or
  • Master’s or higher-level degree as described above.
  • An equivalent combination of education, training, and/or experience.
  • A valid Florida Driver License, in good standing, will be required to travel to offsite locations and various trainings and meetings.
  • Assignment to work a variety of work schedules including compulsory work periods in special, emergency, and/or disaster situations.
  • Ability and willingness to travel throughout Pinellas County.
  • Participation in the Pinellas County Employee Disaster Preparedness & Response operations.

Highly Desirable

  • Knowledge of services offered by the County and other local community services agencies.
  • Fluent in any languages other than English, particularly Spanish.
  • Ability to work a flexible schedule when needed.
