Pinellas County Human Services’ Family Housing Assistance Program (FHAP) is a Housing First, Rapid Re-housing program that provides a pathway for families experiencing homelessness to obtain permanent housing through individualized case management, time-limited financial assistance, and targeted supportive services to foster maintenance of stable housing.

This Human Services Case Manager 2 (Case Manger 2 – Family Housing Assistance Program) position is located at:  647 1st Avenue North, St. Petersburg, FL  33701. Work Schedule:  Monday - Friday, 8am - 5pm, Occasional evening or weekend hours may be required. Position may be approved for partial remote work schedule in accordance with County Policy after an appropriate period of satisfactory performance.

 

What You'll Do:

  • Case Management: Provide long term case management services for clients in your caseload (caseload size is 15-20). Utilize your skills to assess, plan, coordinate, monitor and advocate for the needs of clients. Be a guiding force in connecting families with vital resources and support. Conduct field visits to meet with clients in their homes, shelters, and/or in the community. 
  • Assessment and Planning: Conduct thorough assessments utilizing the Family Service Prioritization Decision Assistance Tool (F-SPDAT) to identify client needs, strengths, and goals. Develop individualized client service plans in collaboration with clients and interdisciplinary teams. Maintain open channels of communication to foster proactive problem-solving.
  • Coordination and Collaboration: Build relationships with families. Work closely with internal teams and external partners to coordinate services, facilitate payments and create a seamless support network for clients. Work with clients to locate and secure appropriate housing; assist with landlord negotiations, housing applications, and leases when needed for prospective tenants.
  • Monitoring and Evaluation: Regularly review and update client progress, adjusting plans as needed. Monitor service delivery, ensuring it aligns with established goals and objectives.
  • Client Empowerment: Empower clients to navigate challenges and build resilience. Foster a supportive environment that encourages growth and positive outcomes. Educate clients on available resources, self-management strategies, and empowerment techniques to enhance their ability to navigate and work through challenges independently. Examples include, but are not limited to:
    • Budgeting and financial literacy skills.
    • Basic life and home care skills; provides training, if needed.
    • Job search, resume development, and interview skills.
    • Builds connections with potential employers, educational institutions, and training programs.
       
  • Documentation: Verify documentation and information provided by clients. Maintain accurate and comprehensive records, documenting assessments, interventions, and outcomes. Ensure compliance with organizational standards and legal requirements. Adhere to confidentiality and privacy standards. 
  • Advocacy: Be the voice for those who need it most. Advocate for clients' rights and access to essential services within the community.

What You Need To Have

Experience:  Technical and professional experience in social casework related to client eligibility determination for housing and social services

Degree:  Social Work, Human Services, or a related field.

  • Six (6) years of experience as described above; or
  • An Associate's degree and four (4) years of experience as described above; or
  • A Bachelor’s degree and two (2) years of experience as described above; or
  • Master's or higher-level degree and some experience as described above; or
  • An equivalent combination of education, training, and/or experience.
  • A valid Florida Driver License, in good standing, will be required to travel to offsite locations and various trainings and meetings.
  • Assignment to work a variety of work schedules including compulsory work periods in special, emergency, and/or disaster situations.
  • Participation in the Pinellas County Employee Disaster Preparedness & Response operations.

Highly Desirable

  • Bachelor's Degree in Social Work, Human Services, or a related field.
  • Bilingual with demonstrated full fluency in written and spoken Spanish.
  • Strong communication and interpersonal skills.
  • Critical thinking and problem-solving skills
  • Knowledge of services offered by the County and other local public and private community service agencies.
  • Proficiency in Microsoft Office Suite (Word, Excel, Power Point).
