Are you a dedicated healthcare professional with a passion for patient care and teamwork? Yakima Valley Farm Workers Clinic (YVFWC) is looking for a charismatic and skilled APC In-Basket Provider to join our dynamic team. This unique role offers a perfect blend of direct patient care and collaborative support for our provider team.

Position Overview:

As an APC In-Basket Provider, you'll balance your time between direct patient care and assisting a dedicated team of providers with EPIC In-Basket management. Your expertise will be crucial in ensuring our patients receive exceptional care and our team functions seamlessly. Our collaborative approach to patient care means you'll be part of a passionate team committed to community health. You'll have the opportunity to build and nurture patient relationships by managing your own patient panel.

Key Responsibilities:

Direct Patient Care (50%):

  • Obtain patient histories, conduct exams, order and interpret diagnostic tests, and formulate treatment plans.
  • Listen to patients to ensure satisfaction and manage continuity of care, including follow-up activities.
  • Order and perform lab and radiology tests, diagnose conditions, and prescribe medications and therapies.
  • Work with the healthcare team to provide standard medical care.
  • Perform minor surgeries and educate patients about their health.
  • Encourage preventive health measures and collaborate with various healthcare professionals for comprehensive care.
  • Provide safe, effective, and patient-centered care, adhering to evidence-based guidelines.
  • Ensure compliance with QA and regulatory standards.
  • Maintain patient confidentiality and accurate documentation in the EMR.
  • Meet clinic hours and adjust to patient volumes.
  • Participate in after-hours call rotations as needed.

EPIC In-Basket Management (50%):

  • Monitor and respond to In Basket messages and items for the assigned team.
  • Handle pharmacy refill requests and prior authorizations.
  • Manage nursing advice messages.
  • Direct patient messaging via MyChart or Artera.
  • Review and respond to both normal and abnormal lab results.
  • Review incoming medical documentation, including Emergency Room or inpatient admission records.
  • Review specialty consult reports and imaging reports.
  • Address other In Basket items as assigned.
  • Keep assigned providers informed about significant patient issues identified during In Basket management.
  • Provide clinical care support for patients assigned to team members, including scheduled routine visits and same-day appointments for urgent care.

Position Details:

  • Proudly serving a largely migrant and Latino community
  • Certified bilingual Medical Assistant dedicated to assisting in translation and patient care
  • 1.0 FTE
  • EMR system is EPIC
  • Clinic Hours: Monday – Friday, 7:45 AM - 7:00 PM; Saturday, 8:45 AM – 5:00 PM

Position highlights:

  • Base Compensation: $132k - $144k.  Additional earnings from productivity and patient satisfaction measures should be expected.

  • $15k Hiring Bonus

  • $20k Relocation allowance

  • $50k student loan repayment assistance beginning the month after your start date

  • Malpractice insurance coverage (no tail coverage policy needed)

  • 4-week Medical Spanish Immersion Program in Xayla, Guatemala

Qualifications:

To be considered for this position, you must complete a Master's or Doctoral Degree -Nurse Practitioner or Master's Degree - Physician Assistant or Bachelor's Degree in Health Science and a two-year Physician Assistant training program.
