Licensed Practical Nurse (LPN)

Job Requirements:
- Current State License Practical Nursing License
- Current CPR (cannot be online course)
- Current Driver's License
- Physical within 1 year (must include state "free of communicable diseases")
- Current TB Skin Test (within 12 months), Chest X-ray (must be within 5 years), QuantiFERON (within 12 months)
- Two Character reference (must be co-workers, not supervisors)
- Current Alzheimer's Training (must be 2 hours)
- HIV/AIDS CEU (Required only if you have held your nursing license for 2 years or longer)
- Proof of full COVID vaccination (per CMS vaccine mandate) OR submit an approved medical exemption (with supportive documentation) OR religious exemption prior to hire date
Duties and Responsibilities:
- Provides skilled nursing care within the nursing scope of practice, the standard of care, and as ordered on the plan of care and in compliance with physician orders.
- Supervised visits by a Registered Nurse within the patient's home at least every 30 days.
- Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures. Documentation is reviewed by Director of Nursing (DON) for approval.
- Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continuned care, to assure the patient care needs are met.
- Addresses and supports patient/family cultural practices, as long as such practices do not harm others or interfere with the patient's treatment plan.
- Promotes and provides patient/family/caregiver education using various verbal and written communication.
- Knowledge of federal, state regulations and Agnecy's policies and procedures regarding patient care.
- Communicates with all members of the health care team, including physicians appropriately and in a timely manner.
- Notifies the physician, the Clinical Director, and other members of the health care team of any changes in the patient's condition and the need to modify the plan of care.
- Maintains currency of knowledge and skills.
- Participates in the Agency's Quality Improvement Program.
- Attends Agency meetings and in-service programs as required.
- Obtains appropriate number of continuing education credits to maintain active license status.
- Maintain confidentiality of patient and Agency information at all times.
