Position Highlights
What You’ll Do
The Health Education Class Facilitator is responsible for the recruitment and enrollment of patients to attend health and wellness education classes, as well as the facilitation of these classes that help patients manage their chronic conditions. Facilitates online and in-person classes at assigned geographic locations covering topics that include Chronic Disease Self-Management Program (CDSMP), “Tomando Control de su Salud (Tomando),” Diabetes Self-Management Program (DSMP), and “Manejo Personal de la Diabetes (Manejo)”.
